Purpose

The purpose of this award is to recognize members who have made extraordinary commitments to ACHCA's mission.

Eligibility

• ACHCA member in good standing for a minimum of 2 years

Nomination Requirements

You must submit two letters - A letter of nomination from the nominator + one letter of recommendation. The letter of recommendation must include the writer's name, address, phone number, e-mail, and the name of the nominee.

The nominator's letter must address this question:

• How has the nominee made extraordinary contributions of time and effort to advance the mission of ACHCA?
