Purpose

The purpose of this award is to recognize an ACHCA member who has been a practicing administrator for five years or less, and who have demonstrated exceptional commitment and potential as a leader, innovator, or a motivator in post-acute and aging services as well as in ACHCA.

Please note: All online nomination forms must be completed in one sitting. You will not be able to save your form to complete it at a later time. If you start the form and close your browser window without submitting, your work will be lost.

Eligibility

1. ACHCA member in good standing
2. Currently licensed administrator
3. Has practiced as a Nursing Home Administrator for five years or less at the time of the nomination, OR
4. Practiced as an Assisted Living Administrator for five years or less at a time of nomination

Nomination Requirements

You must submit two letters - A letter of nomination from the nominator + one letter of recommendation. The letter of recommendation must include the writer's name, address, phone number, e-mail, and the name of the nominee.

The nominator's letter must address these questions:

1. How has the nominee improved the health and welfare of residents in a nursing home or assisted living community?
2. How has the nominee contributed to, or set an example in, post-acute and aging services?
3. What potential does the nominee show for leadership in ACHCA?
4. Why is the nominee deserving of the award?
