Purpose

The purpose of this award is to recognize an individual or organization for excellent journalistic achievement representing fair and accurate content related to the post-acute and aging services industry during the twelve months prior to the submission of the award nomination.

Please note: All online nomination forms must be completed in one sitting. You will not be able to save your form to complete it at a later time. If you start the form and close your browser window without submitting, your work will be lost.

Eligibility

Eligibility for the Journalism Award is solely based on submitted documentation.

The nominee does not have to be a member of ACHCA.

Nomination Requirements

Submit two letters - A letter of nomination for the nominator + one letter of recommendation. The letter of recommendation must include the writer's name, address, phone number, e-mail, and the name of the nominee. Additionally, the nominator should submit two to four examples of the nominee's work (e.g., television and radio program recordings, newspaper or magazine articles, blogs, photo essays, etc.). Examples should not exceed ten total pages and will not be returned.

The nominator's letter must address the following question:

• How does the nominee's work represent fair and accurate content related to the post-acute and aging services environment?
• How has the nominee contributed to the integrity of the post-acute and aging services field?
• Why is the nominee deserving of this award?
