Purpose

The purpose of this award is to recognize a Fellow of ACHCA who has demonstrated the highest professional standards as an administrator in post-acute and aging services, specifically within the assisted living setting.

Please note: All online nomination forms must be completed in one sitting. You will not be able to save your form to complete it at a later time. If you start the form and close your browser window without submitting, your work will be lost.

Eligibility

• ACHCA Fellow in good standing
• Has practiced as an assisted living administrator for a minimum of five years
• Participation in National ACHCA conferences within the past five years preferred

Nomination Requirements

You must submit two letters of recommendation - A letter of nomination from the nominator + one letter of recommendation. The letter of recommendation must include the writer's name, address, phone number, e-mail, and the name of the nominee.

The nominator's letter must address the following question:

• How does the nominee demonstrate excellence in an area of assisted living including facility management, corporate leadership, academia, research, publishing, or other noteworthy activities.
