2027 ACHCA AWARDS NOMINATION

Purpose

The purpose of this award is to recognize a chapter or district that has developed and implemented an outstanding project addressing a member need or chapter/district objective.

Directions

  1. Select a project developed by the chapter/district to address a member need or chapter/district objective. A chapter/district may be nominated for multiple programs—simply complete a separate application for each nomination. Suggested areas to consider include (but are not limited to):
    • Membership Recruitment (% increase)
    • Member Retention Program (% increase)
    • Member Communication (increase in frequency/quality)
    • Student involvement (% student member increase; student chapter affiliation/growth; innocation student programming, etc.)
    • Cultural Diversity Membership Recruitment (% increase)
    • Educational Offerings (increase in # of offering; increase in # of participants; innovative education offerings, etc.)
    • Affiliate Member Recruitment (# or % increase)
    • Vendor Relations (# or % on increase in sponsors or sponsorship revenue; innovative vendor relations, etc.)
    • Leadership Development initiatives (AIT program development; Chapter/district development, etc.)
    • Professional Advancement (% or number increase of ACHCA Fellows or Certified administrators)
    • Public Awareness, Outreach or Service (indicate measurable success)
    • Other chapter/district initiatives that align with ACHCA's mission/vision
  2. Chapter/District Award Applications may be submitted anytime, however, the deadline to submit your Chapter/District Award Application is January 14, 2027.
  3. The Awards Committee will review all Chapter/District Award applications and notify winning chapters no later than 45 days prior to Convention.
  4. A chapter/district representative from the winning chapter/district must attend Convention to receive the Chapter/District Award.
  5. It is suggested that chapters/districts selected for this recognition be prepared to share their innovative/exemplary practice with other chapters/districts at a chapter/district development activity or other educational event.

Nomination Materials

If you have supportive documentation or photos, please upload it in the form linked below. Please use as high quality for images as possible.

Submit a letter of nomination outlining why the chapter or district is deserving of an award for its project or initiative (please be specific). Include at least one additional letter in support of this nomination. The letters must include the writer's name, address, phone, e-mail and include the name of the chapter or district. In the letters, address these questions:

  • What is the name of the chapter or district initiative?
  • Describe how the chapter executed the initiative - who was involved, what steps were taken, etc.
  • What impact has the initiative had on the chapter, its members, and/or other stakeholders? Metrics or numbers are not required but preferred.

Additional Questions to be completed on nomination form:

  • Name of Project or Initiative
  • What is the goal of the project or the initiative?
  • What steps were taken for this project or initative?
  • Was this initiative successful? What was the outcome of this initiative? Supporting metrics are not required but do strengthen nominations.
  • Identify the purpose of the chapter/district initiative. Why did you undertake the project? Describe the situation that prompted it, including any metrics or statistics that provoked the effort.
  • Who are the participants (leaders and targets) of the project?
  • What are the goals and measures? These should be focused, measurable and have a timeframe for completion. Indicate a numeric measure that indicated achievement, (how do you know this initiative will make a difference?).
  • What will be done/was done and by whom (what activities, systems and processes were initiated or changed) to achieve this goal?
  • What was/is the timeline?
  • What has changed as a result of the achievement of this goal (metrics/numbers) and how does this compare to baseline numbers? While not required, metrics/documentation are strongly preferred and will be used in consideration.
  • Has the goal been met? Why or why not? Evaluate the outcome. Determine next steps to achieve better results or to sustain the achievement.
